The Women's Health Initiative (WHI) study found an increased risk of heart attacks, stroke , breast cancer , blood clots , and pulmonary emboli (blood clots that lodge in the lungs ) in postmenopausal women (50 to 79 years of age) who took medroxyprogesterone in combination with estrogens for 5 years, as well as an increased risk of dementia in the women over age 65. Therefore, medroxyprogesterone should not be used for the prevention of heart disease or dementia . Although medroxyprogesterone alone has not been demonstrated to promote breast cancer , since breast cancer has progesterone receptors, physicians usually avoid using progestins in women who have had breast cancer .
Longer duration of treatment and smoking were associated with less recovery of BMD following the last injection of Depo-Provera CI. Table 6 shows the extent of recovery of BMD up to 60 months post-treatment for adolescent women who received Depo-Provera CI for two years or less compared to more than two years. Post-treatment follow-up showed that, in women treated for more than two years, only lumbar spine BMD recovered to baseline levels after treatment was discontinued. Subjects treated with Depo-Provera for more than two years did not recover to their baseline BMD level at femoral neck and total hip even up to 60 months post-treatment. Adolescent women in the untreated cohort gained BMD throughout the trial period (data not shown).
The recommended dose of Depo-Provera is 150 mg every 13 weeks administered by deep intramuscular (IM) injection in the gluteal or deltoid muscle. Depo-Provera should not be used as a long-term birth control method (longer than 2 years). Depo-Provera may interact with aminoglutethimide (Cytadren). Other drugs may interact with Depo-Provera. Tell your doctor all prescription and over-the-counter medications and supplements you use. Depo-Provera must not be used during pregnancy. It may take longer for you to get pregnant after you stop using this medication. This drug passes into breast milk. Consult your doctor before breastfeeding.
