The pancreas is considered a heterocrine gland because it has both endocrine and exocrine gland functions. Small masses of endocrine cells known as pancreatic islets make up around 1% of the pancreas and produce the hormones insulin and glucagon to regulate glucose homeostasis in the blood stream. The other 99% of the pancreas contains exocrine cells that produce powerful enzymes that are excreted into the duodenum during digestion. These enzymes together with water and sodium bicarbonate secreted from the pancreas are known as pancreatic juice .

During minor illness (., flu or fever >38° C [° F]) the hydrocortisone dose should be doubled for 2 or 3 days. The inability to ingest hydrocortisone tablets warrants parenteral administration. Most patients can be educated to self administer hydrocortisone, 100 mg IM, and reduce the risk of an emergency room visit. Hydrocortisone, 75 mg/day, provides adequate glucocorticoid coverage for outpatient surgery. Parenteral hydrocortisone, 150 to 200 mg/day (in three or four divided doses), is needed for major surgery, with a rapid taper to normal replacement during the recovery. Patients taking more than 100 mg hydrocortisone/day do not need any additional mineralocorticoid replacement. All patients should wear some form of identification indicating their adrenal insufficiency status.

We already discussed what happens in your body while you sleep. The good news is, you can help to prevent the muscle breakdown that normally occurs with the help of the slow-digesting protein casein. Just be sure the casein you choose lists micellar casein first on the ingredients list. This is the slowest digesting protein you can get; it takes a good 7-8 hours, providing your body a slow and steady stream of amino acids all night long. Your body will use these amino acids for fuel instead of breaking down the aminos from your muscle fibers. Also, get in one final dose of fish oil at this time. Fat will help to further slow down digestion rate, and the omega-3s will provide a host of other health benefits.
